Go to your MachForm admin panel. If you installed it yourself, it’s probably www.yourwebsite.com/machforms/. If you had it installed for you, the folder name is whatever you requested it to be, so maybe www.yourwebsite.com/forms/ or something similar.
Enter your username and password.
Click on create new form.

Click on Untitled Form to enter your form name and description.

You can also opt to let users preview their info before finally submitting it and change the message they receive once the form has been submitted.

Click on Add a Field at the top of the right column and then choose what time of field you want to add. It will appear on the left. You can do one at a time or all your fields at the same time and then edit them later.

When you click on each field on the left, it will show you options on the right. 1. Enter the field label (what you want it to be titled…Name, Address, etc.). 2. Check the required box if you want them to be required to enter certain fields. This is good for name, email address, etc. 3. Add in any guidance or suggested answers you want the user to have in filling out a certain area. This will show up in a small gray box on the right when they click on that field. 4. When you’ve entered all your fields, click on Save Form.

You will receive confirmation at the top that your form has been saved. Not done yet, though! Click on the Emails icon. If you don’t set this part up, you won’t get notified when someone has filled out a form.

Click on the More Options link.

This will bring up a list of all the variables in your form. Find one for either Name or Email and copy the element name for it. (Ex. {element_1})

Go back to the email form (you can just hit the back button) and enter Your Email Address. Then in the From Name box, paste the element you just copied. Now, when the forms come to your email, instead of being listed as coming from MachForm, they’ll have the name or email address of the person filling out the form. Scroll down and click on Save.

Now you’re done! Click on the View Form icon to see your form. When previewing it, you can also copy the url and use that to give people the link to your form. You can also use the Embed Code icon to create different code to insert into pages if you feel comfortable doing that.

Customizing the form colors and layout will have to wait until the next tutorial. :)
